Please be specific about the reason why your child will not be attending school today. If your reason for submitting a student absence is related to sickness, in the "Reason for Absence" blank space on the form below, please indicate whether your child has any of the symptoms listed.

You may submit this absence in either English or Spanish.

This attendance form is to be submitted by the parent/legal guardian only.

Reason for Absence

Please leave a specific message about why your student is not attending today. Be sure to include symptoms if they are ill, especially if they have fever, nausea, vomiting, sore throat and/or cough. If your child has a diagnosis from a health care provider, please include that information.

If reporting an ill child, please identify any of the symptoms below.

Feeling feverish or chills | Fever of 100.4 F or higher | Headache | New muscle pain | A new cough | A new sore throat | Shortness of breath or breathing difficulties | New loss of taste, smell, or appetite | Congestion or runny nose | Fatigue (more tired than normal or sudden onset) | Abdominal pain | Nausea, vomiting or diarrhea
