Change in Transportation Form

Parent/guardian can complete this form to request a change in routine transportation.  All online requests should be made by 12:00 p.m.  For any changes or emergencies after 12:00 pm, please contact the school's main office at 703-645-6600.

  • Form response will be forwarded to the classroom teacher indicated. If you have multiple Shrevewood Elementary School students for whom this change applies, you must complete this form for each child. 
  • When a student is going home with another student, it is expected that parents have coordinated plans beforehand. Each family must submit the online form by 12:00 p.m.
